For this fax number, you should also put ATTN: ART Team on the cover sheet.

I have also seen good advice on this website that you should always put your name, loan number, property address, and the last four digits of your SSN on each page of any fax you send to help make sure the information is kept together by the recipient of the fax. I have also gotten in the habit of numbering the pages of the fax with Page 1 of 8, Page 2 of 8, etc.
