Difference between Hardship Letter and Hardship Affidavit

I just got a Mod packet from IndyMac. They want a Hardship Affidavit. Whats the difference between that and a hardship letter? Any advice on how to word it?

The hardship affidavit is a form you will need to fill out for the HAMP program.

Any suggestions on how to fill it out? Should it be in letter form or list form?

I'm not exactly sure how to fill out the bottom. I believe it should be the same as your hardship letter..

Any help or suggestions would be appreciated especially from Evan or Moe. Chase/NACA needs me to write another hardship letter showing how /why my hardship is different from one a few years ago when chase did a modification for me (no lowering of rate ,just brought me current) . The change basically was that was a temporary drop in income for a few months compared to a fifty percent drop in income over the past year. would that be sufficient to just say that??? Any help/suggestions/wording would be helpful. Thank you.
