What documents do I need to apply for a mod?

I was wondering exactly what paperwork is needed to apply for a modification? I have my 2008 tax return on hand but I have gone through a disagreement with my bank that caused me to change banks last week. All of my statements from my old bank were e-statements only and now I can't access them. Is this going to pose a problem when I contact my lender (Flagstar)? I'm pretty sure I'm a good candidate apart from that (I hope and pray). Anybody dealt with Flagstar since the guidelines came out on on March 4th? Thanks in advance for any information.

With Flagstar they ask that you call into them for help with the documents............

This is right from their FAQs...............

What information will I have to provide to take advantage of one of the relief options?

We will need to see information that verifies your financial hardship. This might include lay off notices or other notices from your employer concerning your rate of pay or a reduction in hours, tax returns, medical bills and/or other information that documents your financial hardship. We may also need to see recent bank statements, a listing of what you own and what you owe and information on your household budget. We can work with you to assemble this information.


I don't understand the forms required to take advantage of the relief options, can someone help me complete the forms?
