notarized letter - Loan Modification Forum

I am just wondering if anyone who recieved there package got the part where it has to get notarized. I brought it to the notary and they were asking me what goes on the part where is says before ______ Also do they only have to sign in one area. If someone could get back to me I would appreciate it before I put this in the mail. Thanks guys!!

also there are two parts for notary but on the second page theres nothing on it but signature for notary do we need two different notaries. There was only one tab for notary to sign.

The lady whom notorized my modifiation papers said that she had to do it in two spots for the two different names, MY NAME and MY Husband's name. That's why there are two spots, for two names.
